• What is Transition from School to Adult Life?

    Transition is a formal process of long-range cooperative planning that will assist students with disabilities to successfully move from school into the adult world.  High quality transition planning and services will enable students with disabilities to pursue their desired postsecondary goals. 



    A transition plan describes a course of study and related strategies and activities based on the student’s strengths, interests and preferences, to assist the student in attaining postsecondary goals related to training, education, employment and/or independent living. The transition plan must be included in the IEP.
