What forms are do I need for my child?

Required forms are listed below by the grade your student is entering. Annual required forms and laptop and activity fee must be submitted for each student through Community Pass.

All optional forms are listed by the grade students are entering in the fall.  Please be mindful of which forms are to be completed and which are to be downloaded, printed out, and returned to school. Forms must be completed, and payments received in Community Pass prior to the start of the upcoming school year. 


Entering 7th and 8th Grade


Complete in CommunityPass:

  • Annual Student Form (required), laptop and activity fee

  • Annual Student Form (required), laptop and activity fee
